No Show Policy: As a courtesy to our patients and office, it is important you keep your scheduled appointments. A no show means a loss to us and another patient to receive treatment. Therefore, after three (3) consecutive no shows, we will refer you elsewhere for your mental health needs.

Phone/Electronic Device Policy:  If you wish to listen to music/watch videos on your electronic device, a headset is required. For patient privacy, no photos are to be taken in our office.

To better serve our patients, we have implemented important office policies and procedures regarding your treatment with Dr. Charles E. Gutierrez. Please read them carefully. If you have any questions, please raise them with your doctor or our office staff.

Our Office Policies

Food Policy: During your visit, food or drinks are allowed in our waiting area.

Appointments: Patients are seen by appointment only. For the first appointment, you will need to bring the following items: patient's insurance, patient's Social Security card and ID (if the patient is a child/adolescent, bring the parent/guardian's ID). If the child/adolescent is adopted, bring the signed adoption papers. If you have legal custody of the child/adolescent, bring the signed court papers. Additionally, bring your completed new patient paperwork, as this will help reduce your wait time.

  • Patients who complete their paperwork first, may be seen first.
  • The first appointment will vary in time of 30 to 60 minutes.
  • The second appointment (Testing) varies dependent on the age of the patient 1 to 4 hours.
  • The third appointment (Results) time varies from 20 to 45 minutes.

Emergency Call Services: In case of an emergency, please go to the nearest hospital or dial 911, as Dr. Gutierrez does not provide 24-hour service.

Child, Adolescent & Adult Psychologist

Cancellation Policy: If you need to cancel an appointment, please notify our office as soon as possible.
A missed appointment without 24-hour notice will be charged as a session and you will be charged a $75.00 "no show" fee.

Payment of Fees: Payment is appreciated at each visit. We will assist in completing your health insurance claim; however, the CLIENT, NOT THE INSURANCE is responsible for payment of the bill. If another arrangement is needed, please consult with our staff. All efforts will be made to work out an acceptable method of payment. If the client fails to comply with attempts

to negotiate payment, Dr. Gutierrez's office will utilize an outside collection agency for delinquent accounts.
